Insurance Information

Mystic Valley Urology is a provider for Medicare and most major medical insurance plans. If you have any questions about your insurance please call 781-979-0661 and ask to speak to a member of our billing department, they can assist you in determining if we accept your insurance. Any questions about your insurance coverage and benefits should be directed to your employer or insurance company.

You are responsible for any charges that are not covered by your insurance at the time of your visit including your co-pay, deductible or any other out-of pocket expense. If your insurance requires a referral from your primary care physician, we must have this prior to the date of your appointment. It is your responsibility to obtain the referral, failure to do so will result in rescheduling your appointment.

If you have no insurance or if your procedure is not a covered benefit, you will be expected to pay at the time of your visit. Please call 781-979-0661 and ask to speak with a member of our billing department, they can assist you with an estimate of charges.

For your convenience we accept Visa, MasterCard and Discover in addition to cash and personal checks.

Medical Records

All medical records are property of this office. We will not release any information to another physician, an insurance company or an attorney without a written authorization. A signed “Authorization to Release Medical Records” form will be kept on file.

Prescription Refills

It is always best to ask your provider for a prescription refill at the time of your appointment. Should you need a refill prior to your next visit, please call your pharmacy and then will send us the request electronically to our new electronic medical record system. Please note; allow 48 hours for your refill to be processed. If you have not seen your doctor in more than one year, you will need to schedule an appointment before your refill can be processed.

Lab results

A urology practice processes a large number of varied laboratory tests on a daily basis. Because of the large number of specimens we do not routinely call you with your test results. Often, you will be asked to schedule a follow up appointment for you to see your doctor and he/she will review your results at that time. If you are wondering about your results you may call the office to obtain your results. Please be aware that some tests take at least 48 hours before results are available and others take up to a week or more. When requesting lab results, be certain to give your name, age, the test performed and the location of the laboratory you visited.
